IMMUNIZATIONS
All children must be immunized as per DHS regulations. All immunization records must be kept in your child’s file for the duration of enrollment. Health appraisals are required for infants and young toddlers no less than every 6 months and annually for ages 2 - 5 years. Once 6 yrs old and in first grade, health assessments are required every two years.
Health assessments and immunization records are required within 30 days of enrollment.
Forms are located in our classrooms on information bulletin boards and in the office.
